 Here's what you need to do:

• Find a Care Coordination Organization (CCO) in your county: These local organizations can guide you through the application process. Find a CCO in your county on the OPWDD website.
  • CCOs can help you gather information and documents needed for the application. These documents typically include recent evaluations, medical reports, and proof of age of onset for people over 22.
  • They can also submit your application electronically to OPWDD and track its status.
