Getting Started - Submission Confirmation
Thank you for reaching out to us about services at YAI. Your submission will be reviewed by staff in YAI’s LINK department, and someone will be in touch with you within five business days to follow up. A confirmation email should also have been sent to you, check your spam if you don't see one.
If you were reaching out to us about a referral for medical services through Premier HealthCare, including appointment scheduling for services or COVID vaccines, or test results, please call our Patient Services department at 212.273.6272.
Please be aware that we are unable to handle urgent requests via web form, phone, or email. If this is a medical emergency, please dial 911 or go to your nearest emergency room.
