There are significant upcoming changes to New York State’s Medicaid Service Coordination (MSC) service commencing July 1, 2018, and introduce you to Care Design New York, LLC (CDNY), one of six entities within New York State designated as a Care Coordination Organization / Health Home (CCO).
CDNY is a partnership of several I/DD agencies that operate throughout OPWDD Regions 3/4/5. Starting this summer, the CDNY member agencies -- all of which provide quality, person-centered supports to people with intellectual and developmental disabilities (I/DD), will be transitioning their MSC service and Medicaid Service Coordinators over to CDNY.
CDNY, with an anticipated enrollment of more than 25,000 people, will likely be the largest and most influential CCO in NYS. The fact that YAI is a 15.25% equity partner of CDNY and that I'm a member of CDNY's executive management committee and board of managers, means that YAI has secured a seat and meaningful voice at an important table that will play an influential role during our field's multi-year and multi-step transition to managed care, value-based payments, and beyond.
As of July 1, to comply with a CMS mandate requiring conflict free case management, MSC service will no longer be provided at an agency level. Instead, MSC service will be replaced by Care Coordination service and be provided by Care Coordinators at the CCO level.
What does this mean? It means that CDNY will soon start to transition (hire) MSCs from its member agencies. All current MSCs and MSC supervisors will be offered Care Coordinator jobs with CDNY. Care Coordinators will continue to be based within their current geography, and the people we support will have the option of having their current MSC become their new Care Coordinator. (Assuming their MSC opts to transition over to CDNY.) To remain readily accessible, CDNY will operate regional hubs.
How is Care Coordination different? Care Coordination is designed to enhance the current MSC service by adding a physical and behavioral health coordination component to the traditional MSC service.
